Measuring oral temperature

Steps

1. Wash your hands.

2. Put on disposable gloves

3. Prepare all required equipment's.

- Thermometer

- Cotton

- Alcohol

- Kidney basin

4. Explain the procedure to the client

5. Take the thermometer and wipe it with cotton

sponge from the bottom to tip.

6. Tell the patient to open mouth and place the

thermometer under the tongue for three minutes

and record as it is.

7. Instruct the patient to close his lip.

8. Remove and read the level of thermometer at

eye level.

9. Replace all equipment's in proper place.

10. Record in the client’s chart and give

signature on the chart


Measuring axillary temperature

Steps

1. Wash your hands.

2. Put on disposable gloves

3. Prepare all required equipment's.

- Thermometer

- Cotton

- Alcohol

- Kidney basin

4. Explain the procedure to the client

5. Take the thermometer and wipe it with cotton

sponge from the bottom to tip.

6. Move clothing away from shoulder and arm

7. Place the bulb of thermometer in hollow of

axilla for five minutes and recorded after

increase half-degree .

8. Keep the arm flexed across the chest.

9. Remove and read the level of thermometer at

eye level.

10. Replace all equipment's in proper place.

11. Record in the client’s chart and give signature

on the chart.


Measuring rectal temperature

Steps

1. Wash your hands.

2. Put on disposable gloves

3. Prepare all required equipment's.

- Rectal thermometer

- Cotton

- Alcohol

- Kidney basin

- Lubricate

4. Explain the procedure to the client

5. Take the thermometer and wipe it with cotton

sponge from the bottom to tip.

6. Lubricate the end of the temperature probe

7. Gently insert the temperature into the rectum

for 1 minutes and recorded after decrease half-

degree

8. Remove and read the level of thermometer at

eye level.

9. Replace all equipment's in proper place.

10. Record in the client’s chart and give signature

on the chart.
